    Please note:
    A name or contact details are not required. However, it is recommended to include your name and a method of contact to ensure your feedback is given proper consideration and follow up. This will only be done as/if deemed appropriate/necessary by the LVDL committee.

    Feedback should not be used to attack, insult or belittle the person(s) you are reporting, misuse of feedback may result in invalidation of your concerns or even reprimand.

    Feedback submitted is accessible by ALL Executive Committee members. If you have something of a sensitive nature, you may choose to contact a committee member directly and they will be able to assist you. (You may use this form to request a specific member to make contact with you).

    ANY FEEDBACK BEING SUBMITTED BY PLAYERS RELATED TO A GAME THEY HAVE PARTICIPATED IN, MUST BE SUBMITTED TO THE COMMITTEE VIA THEIR TEAM CAPTAIN.​
    You may check the "FYI" box above if you do not require follow up from us, but just want to officially inform us of something.  (Please note: Not checking the box does not guarantee follow up, this will be at the executive committee's discretion).

report  a  serious  incident  but  uncomfortable  reporting  to  the  LVDL?

If you need to report an incident relating to harassment, violence, discrimination, assault, sexual harm, stalking or other unwelcome/offensive behavior (and do not feel comfortable reporting this directly to the  LVDL) you may report this via the Australian Dodgeball Federation.
Please be aware that this is not to be used to report on game-play, refereeing or rules disputes.
Intentional misuse of this form is not acceptable.
